Supporting your child during the school day
If your child has a medical condition we can support them more effectively if we have all the relevant information. The Individual Health Care Plan is the document we use to gather this information. Please download and complete and return this form if your child has a medical condition that impacts their school day.
Medication
Ideally, medication is prescribed outside normal school hours, however, we appreciate this is not always possible. If you are happy that your child is competent to carry and administer their own medication please download and complete our Medication form – Self Administration. If you feel your child requires a trained member of staff to administer medication to them during the school day please download and complete this parental agreement. Spare emergency medication, such as adrenaline auto injectors or reliever inhalers, can be stored in the medical room on completion of a care plan and the relevant medicine administration form.
Changes in mobility (for example, temporary use of crutches)
Should a medical condition impact your child’s ability to move around school, please inform the Medical Needs Co-ordinator so that a risk assessment can be carried out to ensure your child can be safely evacuated in the event of an emergency.
In all cases, care plans and forms should be marked for the attention of the Medical Needs Co-ordinator.
